I made some accidentat glitch art myself while working on a project a few weeks back.
I was doing a test of php image generation functions for a web painting application and was making random noise images ( <a href="http://24.193.97.94/doodlepad/aaaa_t.png">like this</a>)... but I accidently seeded the number from the time inside the loop each time, resulting in strange semi-random patterns: <a href="http://24.193.97.94/doodlepad/testres.jpg">like this</a>.
Was pretty cool after it I realized the bug (was pretty frustrating before that :) ).

Weird, I was just talking about this to a colleague with a particularly cool visual glitch in our particle system. I was proposing making an archive of the more interesting ones, although it'd be a bit difficult to distribute. Maybe as "out-takes" or for easter-eggs. Unfortunately a large part of my job is to eliminate 'art' like this (and to be honest, 99% of them are in no way interesting).
Failing sound code is my personal favourite. In Kinetica, I had a set of bugs that cancelled and restarted every engine sound every fifth of a second, maxed out their volumes, but preserved pitch and pan.
